CCS provides a variety of home-based supports to individuals with intellectual and developmental disabilities.

The supports we offer through our Home & Shared Living program ensure housing security and maximize independence, often helping individuals avoid nursing care or even homelessness. Supports to individuals range from strengthening current family-based living options, to developing independent-living skills, to 24/7 home support from a shared living provider, contracted by CCS.

CCS provides housing through contracted Home Providers. Last year, CCS delivered home supports for 35 Vermonters. Our Maintenance and Safety Office performed more than 20 home inspections to ensure safety and accessibility standards.

Homeward Service Coordination

While not specifically a crisis care provider, CCS staff are on-call, or directly engaged, in clients’ crisis situations, CCS staff has: 

  • supported individuals at emergency room visits and unexpected hospital stays 

  • responded to an individual having a mental health crisis at home 

  • served on a team helping a client with a drug addiction 

  • provided 24/7 care to clients facing homelessness
